Patients ready to be discharged from the hospital aren't always ready to go home. Lutheran Health Network provides different levels of short-term, post-acute care for patients who need extra recovery time, skilled nursing care and rehabilitation services. The level of care required varies based on each patient's ongoing needs.

Skilled Nursing Care
Skilled care services for patients recovering from illness, injury or surgery are provided at Bluffton Regional Medical Center, Dukes Memorial Hospital and St. Joseph Hospital. Daily care is delivered in a relaxed environment with full hospital services nearby if needed.

These skilled nursing facilities provide:

  • 24-hour skilled nursing care
  • Physical, occupational and speech therapy
  • IV and respiratory therapy
  • Multidisciplinary care
  • Case management services
  • Planned activities

Inpatient Rehabilitation Facility (IRF)
The Rehabilitation Hospital of Fort Wayne is the only hospital in the region dedicated solely to physical medicine and rehabilitation. Patients who have experienced a disabling injury or illness receive focused care to help them reach their maximum potential and regain independence.

Benefits of inpatient rehabilitation facilities include:

  • Ability to manage complex medical conditions
  • Rehabilitation nursing
  • Skilled nursing
  • Minimum of 15 hours of therapy per week
  • Physical, occupational and speech therapy
  • IV and respiratory therapy
  • Physician visits at least 3 times per week
  • Interdisciplinary care
  • Case management services
