Patients & Visitors
We understand the value of visitation for our patients and seek to balance their desire to see family and friends with their healing process.
Visitor guidance:
- Two visitors per patient at a time.
- Clinical areas that care for our most vulnerable patient populations (such as NICU, Oncology, Transplant and others) may require masking.
- Visitors should refrain from visiting if they feel ill or are experiencing respiratory symptoms.
Staff may use their clinical judgement to determine the appropriate number of visitors for each patient, taking into account all aspects of patient health and safety, as well as the impact on other patients.
Patients and team members are welcome to wear a mask anytime they choose. We may also need to adjust masking guidance in the future should circumstances change.
Advance Directives
Each individual has the right to direct his or her own care. Current information about Advanced Directives and Indiana state forms can be found on the Indiana State Department of Health website.
Forms included, but not limited to, include:
- Health care representative appointment
- Life prolonging procedures declaration
- Living will declaration (English and Spanish versions)
- Out-of-hospital do not resuscitate declaration and order
Parental Consent
If you're traveling and have to leave your children in the care of someone else, be sure they're protected. Always leave written authorization so your children can receive medical treatment, if needed.
Simply download the form below, fill it out and leave it with your children's caregiver. An authorized form is valid for one year following the signature date unless the form has been withdrawn with written consent.
