What is Lutheran Health Network MyHealthHome?
MyHealthHome is a free, secure online patient portal that gives you access to your health information anytime, day or night.
Can I access my historical records not included in the current MyHealthHome patient portal?
Yes. Access the FollowMyHealth Personal Health Record (offered by Veradigm) for details about visits prior to:
- Bluffton Regional Medical Center - Aug. 18, 2018
- Dukes Memorial Hospital and Kosciusko Community Hospital - Oct. 13, 2018
- Lutheran Health Physicians locations outside Allen County - Oct. 27, 2018
- Dupont, Lutheran, Rehabilitation, St. Joseph and The Orthopedic hospitals - March 2, 2019
- Lutheran Health Physicians locations in Allen County - April 6, 2019
Can I access the portal from my phone?
Yes. Download the MyHealthHome – Oracle Health app, select "MyHealthHome-Lutheran Health Network" as the organization and sign in just as you would on a computer or tablet.
Is there any way to connect mobile health management apps to my health record?
You may be able to securely connect certain third-party health management apps (i.e. Apple Health, fitness trackers, dietary trackers, etc.) to your MyHealthHome data. If interested, please email lhn-api@lhn.net and include your name and contact information so a representative can contact you.
Enrolling
How do I set up a new MyHealthHome (Oracle Health) account?
There are two simple ways to sign up for MyHealthHome:
- Sign up during a visit to any Lutheran Health Physicians office or during a hospital stay. Watch for an email from noreply@iqhealth.com. Click the enrollment link to complete the process.
- After an office visit or hospital stay, go to https://lutheranhealth.iqhealth.com/self-enroll/ and enter the requested information.
What if I don't receive the invitation email?
To ensure that the MyHealthHome invitation email reaches you, add noreply@iqhealth.com as an approved sender. If you don't receive an email and have checked your SPAM folder, click here to self-enroll.
What is my username? My password?
You determine your own username and password when you create your MyHealthHome account online.
Access
How can I access my child's or other family member's health information?
If you are interested in MyHealthHome access for your child(ren) or an adult for whom you are legally responsible and that individual is a patient of a participating provider, you will need to sign up in person by completing an Authorized Proxy User Access Request Form. Sign up with participating physician offices takes place at the time of provider appointments.
Can I give others access to my information?
Only you - and those you authorize - will be able to see or gain access to the information in your portal.
Password/Log-In Issues
What if I forget my password?
If you don't remember your password, select "Forgot password" and follow the instructions to reset your password.
What if I have trouble logging in or using the new (Oracle Health) portal?
Call MyHealthHome Technical Support at (800) 669-4096.
What if I need help with the previous version of the portal (FollowMyHealth)?
Please email myhealth@lhn.net.
Confidentiality and Privacy
Will my portal information be secure?
Yes. MyHealthHome uses a secure, encrypted connection and all health data is stored on a secure server.
Personal and medical information in the MyHealthHome portal remains confidential. Only you - and those you authorize - are able to see or gain access to the information in your portal. We will not share your personal information with any outside party not affiliated with our organization.
