Next Steps
Once your application has been initially approved, additional documents must also be completed before final approval and placement.
To view additional requirements and documentation or to take the next steps if you have already submitted an application:
Submit all forms and documentation together as a packet to the network student coordinator by sending an email to studentcoordinator@lhn.net.
Finally, please complete a student IT access request form once your application has been initially approved for each student.

When the required documents have been submitted and approved by the student coordinator, a second email will be sent to you, your preceptor and the department director confirming the receipt of the required documents. The required documents must be received by the student coordinator at least one week prior to your first day of clinical rotations or internship.
On your first day, you must report to the student coordinator department to present your government-issued photo ID and be given a facility student badge. You will not receive a badge or perform any student assignments until all required documents and a picture ID have been presented. You will also be required to return your badge on your last day of clinical rotations.
